Intended for Users 18 Years of Age and Older:-

In addition to the Online Patient Services, this Site is intended for use by individuals 18 years of age or older. It is not intended for use by children under the age of 18. Users under the age of 18 must have the assistance of a parent or guardian to use this Site.

Our Online Patient Services are available to patients 18 years of age or older. They are also available to patients between the ages of 13 and 17 with the consent of their parents or legal guardians.
